Wellness Waiver

Acknowledgment of Wellness Services
I understand that the services offered by Transforming by Grace — including wellness experiences, coaching, workshops, and restorative practices — are supportive in nature and are not a substitute for medical, psychological, or psychiatric care. I am participating voluntarily and at my own discretion.
Assumption of Risk
I acknowledge that wellness experiences may involve gentle movement, breathing practices, reflection, and emotional processing. I understand there are inherent risks associated with these activities. I assume full responsibility for any risk of injury, harm, or loss I may incur as a result of participating.
Medical Clearance
I confirm that I am in good physical and mental health and have no condition that would prevent me from safely participating. I understand it is my responsibility to consult a licensed healthcare provider before beginning any new wellness practice if I have concerns.
Release of Liability
In consideration of being permitted to participate, I, for myself and my heirs, release and hold harmless Transforming by Grace, its founder, staff, and affiliates from any and all claims, demands, or causes of action arising from my participation, except those caused by willful misconduct or gross negligence.
Privacy & Confidentiality
I understand that any personal information shared during my experience will be treated with care and confidentiality, and will not be disclosed without my consent except as required by law.
Photography & Media
I understand that Transforming by Grace may occasionally capture photos or video for promotional purposes. My image will not be used without my separate written consent.
Agreement
By signing below, I confirm that I have read, understood, and agree to the terms of this wellness waiver. I acknowledge that I am 16 years of age or older, or that I am the parent or legal guardian signing on behalf of a minor guest.
